Our Enhanced Reablement services provide planned short term, intensive support to users returning home after stay in hospital. The aim is to help support individuals restore confidence and independence by focusing on improving their ability to carry out daily living skills to do as much as they can for themselves, rather than someone performing such task for them. We work in partnership with all medical professionals, involved such as the Discharged teams and Occupational therapists to ensure optimum recovery is fully achieved.
